• To get into the role of the priest, Stellan Skarsgård was inspired by reading the Bible.
• Screenwriter William Peter Blatty (1928-2017) attended the film's premiere with a friend, Paul Schrader, and they laughed so loudly during the screening that they almost got kicked out of the theater.
• When Paul Schrader submitted the finished film to Morgan Creek Productions, he was fired, told that he hadn't filmed what was required. Schrader himself recounted in interviews that he had filmed more of a psychological drama than anything else. Renny Harlin was brought in to replace Schrader and ordered to reshoot the film. He reshot almost 90% of the film and completely cut out two characters. Harlin's version was met with a cool reception from the public and critics, while Schrader's version, titled "Exorcist: The Beginning," was released in 2005.
• In solidarity with the fired Paul Schrader, all actors involved in the film (with the exception of Stellan Skarsgård) refused to reshoot in Harlin's version.
