- The film was originally conceived by John Woo as a prequel to 'A Better Tomorrow', but a disagreement with the producer ruined the project. However, 'Bullet in the Head' retains one scene that directly references the 1986 film: in a nightclub, the heroes are forced to down a bottle of liquor at gunpoint – an episode Mark, Chow Yun-Fat's character in 'A Better Tomorrow', recounted.
- John Woo's original version ran for over three hours. Like in the case of 'A Better Tomorrow 2: Hurricane Fire' (1987), the director was forced to cut the film down to just over two hours. However, according to Woo himself, the version shown in Hong Kong the day before the premiere was 2 hours and 35 minutes long. It was decided, however, that even this was too long, so all versions of the film already sent to cinemas had to be urgently cut and then resent to cinemas. Even after all these editing difficulties, due to local censorship and local market requirements, many different versions of the film of varying lengths exist.
- At the end of the film, in the episode in the underground parking lot, you can see that the moment of the collision with the 'Mercedes' is repeated twice: the 'Mercedes' first moves and receives almost no damage, then immediately a piece is inserted where the car crashes into a parked 'Mercedes', already visibly damaged in the collision.
