• While Don is sitting in the car listening to the radio, an "ambulance" drives by. This was not in the script; an ambulance simply happened to drive past the car where the actress was sitting.
• Initially, the scene was supposed to take place not in a pizzeria, but in a burger restaurant. Director Damien Leone made the corresponding changes to the script when it turned out that Jenna Cannell and Kathryn Korcoran are vegetarians.
• Jenna Cannell insisted that she not be untied between takes. The actress believed that this would help her achieve the necessary psychological state more quickly.
• David Howard Thornton was the sixth actor to audition for the role of Art. During the audition, Thornton began to portray tasting the blood of a decapitated victim, and then salting it. This nuance convinced the filmmakers to give the role to Thornton.
• The underground tunnels shown in the film were part of the abandoned Seaview Hospital. The bodies of deceased patients were transported through these tunnels for cremation.
• The latex mask worn by the clown maniac Art is the same one worn by Mike Jannelli in Damien Leone's horror film "All Hallows' Eve".
