The film is based on John Godey's novel "The Three Worlds of Johnny Beautiful".
Johnny's birthday – September 16th – can be seen on a monitor in the hospital; it coincides with Mickey Rourke's own birthday.
Al Pacino once expressed interest in the lead role and even worked with the producers on the script, but ultimately left the project because, despite numerous rewrites, it always turned out to be a low-budget film.
It took three hours each day to apply makeup to Mickey Rourke.
According to director Walter Hill, he was offered this film three times, but refused and only agreed on the fourth attempt.
