The film is based on the autobiography of Susanna Kaysen, who was sent for "voluntary treatment" to McLean Hospital in 1967.
Rose McGowan and Claire Danes auditioned for the role of Lisa Rowe.
Most of the scenes were filmed in a real psychiatric hospital in Harrisburg, Pennsylvania. The hospital was closed in 2005.
