- The working title of the film was “The Organ Donor.”
- Chris Rock is a longtime fan of the "Saw" franchise and convinced Lionsgate executives to film it.
- According to director Darren Lynn Bousman, one of the gruesome scenes had to be cut from the film so that the Motion Picture Association would give it an R rating instead of NC-17.
